Description
One of my team members created this bug:

Which I applied the diff to my instance, but provisioning is still broken.
If i choose a discovered host, click provision and then add a bonded interface the bond form does not drop down.
Even if i save the host and provision it without a bond, the form doesn't drop down.
However if i edit a host that had a bond prior to 1.13.1 i can see the dropdown but i have other issues reported if i try and edit the bond.
After further testing this is the behavior i see.
Discover host, click provision, choose bond. Create an interface as bond 0 - no dropdown. Click save and allow a broken host to exist. Now go and edit the host and the bond dropdown shows and allows me to select bond type (active/passive) and add the interfaces. Then i have to cancel build and click build again since i can't actually get the form to show correctly during the provisioning - only after the host exists and i can edit it twice.
